Your written plan

GP chronic condition management plans

A GP chronic condition management plan is a written plan prepared with your GP for one or more conditions that have lasted, or are expected to last, six months or more. It records your goals, the treatments and services you need, and when you will be reviewed.

From July 2025 these plans replaced the older GP management plan and team care arrangement items.

Understanding chronic disease management

Plain-language information to help you know what to expect and make informed decisions with your GP.

01

What counts as a chronic condition

A chronic condition is one that has lasted, or is expected to last, six months or more, such as diabetes, heart disease, asthma, COPD, arthritis, kidney disease, osteoporosis, chronic pain or a mental health condition.

Many people live with more than one, which makes coordinated care even more important.

02

MyMedicare and your plan

MyMedicare is a free voluntary registration with your usual practice. It links you to Morisset Doctors and supports ongoing care, including some care plan and telehealth items.

Registering takes a few minutes online.

03

Making goals that stick

Good goals are specific, realistic and meaningful to you, such as walking for 20 minutes five days a week, rather than just losing weight.

Your GP and allied health team can help you break them into small steps.

04

Medicines and polypharmacy

People with several conditions often take several medicines. Regular reviews help avoid side effects and interactions.

A pharmacist-led Home Medicines Review can be arranged by your GP.

Build your care team

Tap the conditions that apply to you to see what a coordinated plan might include. Your GP will tailor it to you.
