Understanding health assessments

Plain-language information to help you know what to expect and make informed decisions with your GP.

01

Why assessments help

Health assessments are structured checks that pick up risks and conditions early, often before symptoms appear.

They end with a written plan and follow-up.

02

Who does what

The practice nurse often collects history, measurements and questionnaires, and your GP reviews the findings, examines you and agrees the plan.

Allow a long appointment.

03

After the assessment

You may be referred for tests, screening, vaccines, allied health or a care plan.

Book follow-up appointments before you leave.
